He accepted that he was lucky to be alive but knew that his life would never feel complete unless he could regain a more natural appearance. By 2020, after exhausting all other medical options, his doctors told him that a face transplant was his only remaining hope. The Mayo Clinic in Rochester, Minnesota, agreed to perform the procedure, marking only the second face transplant in the hospital’s history—and its most intricate one to date. Doctors informed Derek that 85% of his face would need to be replaced, including his forehead, eyelids, nose, mouth, and jaws. The surgery would also require transplanting the donor’s skin, muscles, and nerves, carefully reconnecting them to restore essential functions such as blinking, eating, and even smiling. After months of preparation, including 3D modeling and practice surgeries, Derek and his family anxiously awaited a suitable donor. Nine months later, in February of that year, they received the long-awaited news—a donor had been found.

The 50-hour surgery was an immense undertaking, pushing the limits of modern medicine. When Derek finally woke up, he had to wait a full month before seeing his new face. The moment he did, his mother, Lisa, called it “the most incredible moment of my life.” At first, Derek struggled to believe the reflection staring back at him was really his, but as the days passed, he realized something remarkable—he looked normal again. No longer did people stare or point; he was just another face in the crowd. With his new appearance, Derek regained more than just his looks—he found confidence, hope, and a renewed purpose. Alongside his mother, he became a motivational speaker, using his journey to inspire others struggling with mental health challenges. By sharing his story, he hoped to remind people that even in the darkest times, there is always a path forward.
